The U.S. State Department requires that the passport application be completed online, on their website. We have divided the process into 2 steps.

Applications for those under the age of 16 require that the applicant and BOTH parents be present to sign the application or NOTARIZED Form DS 3053 be submitted from the absent parent. If one parent has sole custody, that parent must submit a legal copy of the court order that grants sole custody or a death certificate of the other parent if that is applicable.
- – Return Shipping Fees should be included with payment to Professional Visa and Passport Services. DO NOT include it on the check to the US Passport Office.
- Step 1, Our instruction packet explains the process to be followed after completing the online application in step 2. It contains our order form, a required authorization letter, and our instructions to assist in obtaining a U.S. passport. You can complete the order form online before printing, or by hand afterwards.
